One of the more challenging diagnostic efforts in dentistry is the assessment of the structural integrity of teeth and dental implants. Cracked teeth, deteriorating/fracturing restorations, loss of cement seal or loss of osseointegration are often hard to diagnose until the results are advanced or catastrophic. Quantitative Percussion Diagnostics (QPD) has been shown in over 28 peer reviewed studies in engineering and dental journals to be an accurate and reliable test for determining the structural wellness of teeth or implants. QPD is a mechanical test similar to evaluation methods used in engineering for assessing structural stability of objects, in contrast to visually based dental diagnostics such as radiographs and visual examination. The mechanism that creates QPD data has been illustrated by Finite Element Analysis of novel models of uncracked and cracked teeth. QPD, FEA modeling plus the addition of AI and Machine Learning are currently creating a future for dentistry of predictive diagnostics, where structural problems can be diagnosed early and rated as to type or severity. Preventive or therapeutic steps can then be instituted earlier in the damage cycle prior to the occurrence of advanced or catastrophic results, providing dentists with a new aid to diagnosis.

      Module 4: Managing Black Triangles and Closing Diastemas
      Closing diastemas with composite resin  is a challenge for all of us. Post orthodontic cases are very prevalent due to a tooth size discrepancy. These require minimally invasive techniques due to the age of the patient. Obtaining proper proximal and gingival contour, adequate width, good proximal contact and blending of the composite resin into tooth structure are of outmost importance for longevity and esthetics. 

      This module will present a detailed step-by-step technique to close midline and other anterior diastemas. It will teach shade selection and very importantly opacity selection, tooth preparation, bonding, matrices, layering, “pull through techniques” and contouring and polishing.

      Module 5: Mastering Direct Composite Resin Veneers
      This module will teach CMO members how to master direct composite resin veneers and offer these restorations to their patients. 

      Mastering the direct resin veneer takes understanding of preparation design, thickness of the different layers and how to use effect shades of composite to create a truly esthetic restoration that can rival porcelain. Using achromatic shades will allow the dentist to provide incisal translucency and create effects that match the natural tooth. 

      When you have a dark substrate how do you handle the value? Using different opaquers and understanding the use of tints will be taught in detail. How deep should you prepare the tooth depends on a number of factors. These will be addressed and tips will be given on how to treat the endodontic tooth. Finishing and polishing for maximum longevity will be shown. How to fabricate a single central incisor veneer is challenging, and you will learn how to accomplish this in an efficient manner.

        There is a scarcity of evidence-based research supporting the use of contemporary ceramics in dental practices today. Poor clinical trials eventually lead to poor systematic reviews, and load to failure laboratory tests have little clinical relevance. This leaves clinicians in doubt about which ceramic restorative material is best for their patients. 

        The presentation will focus on lithium disilicate and monolithic zirconia, including the various generations. This will include a discussion of mechanical and optical properties, wear of natural antagonists, as well as ceramic adjustment, finishing, and polishing. A series of new retrospective studies will also be introduced to evaluate the survival of these ceramics.

            About Dr. Robert Margeas
            Dr. Margeas graduated from the University of Iowa College of Dentistry in 1986 and completed his AEGD residency the following year. He is currently an adjunct professor in the department of Operative Dentistry at the University of Iowa. He is Board Certified by the American Board of Operative Dentistry. He is a Diplomate of the American Board of Aesthetic Dentistry, a Fellow of the Academy of General Dentistry, American Society for Dental Aesthetics and International Team of Oral implantologists (ITI). He has written numerous articles on esthetic and implant dentistry, and lectures and presents hands-on courses nationally and internationally on those subjects. He is currently the Editor in Chief of Inside Dentistry, on the Editorial Board of Compendium, and is a contributing editor to Oral Health in Canada. His memberships include OKU Honor Dental Society, American Academy of Esthetic Dentistry, and the American Academy of Restorative Dentistry. Dr. Margeas maintains a full-time private practice focusing on comprehensive restorative and implant dentistry in Des Moines, Iowa.
            About Dr. Marcos Vargas
            Dr. Marcos Vargas attended Cayetano Heredia University School of Dentistry in Lima, Peru and graduated in 1985. He spent two years, 1990 to 1992, in the AEGD program at the Eastman Dental Center in Rochester, New York. Dr. Vargas received his Certificate and master’s degree in Operative Dentistry in 1994 at the University of Iowa where he is currently a Professor in the Department of Family Dentistry. His primary research interests are in the area of dental materials including glass ionomers, dentin bonding, composite resins and esthetic dentistry. Dr. Vargas is also recognized for his expertise of Direct Restorative Treatment Procedures and conducts numerous lecture and hands-on seminars in the US and internationally. Dr. Marcos Vargas has published extensively in the area of dental adhesion and resin composites for over 25 years. He maintains a private practice limited to Restorative Dentistry with an emphasis on aesthetic dentistry.
